In order to provide you with a professional quality service in natural fertility regulation we will need to ask you personal and sensitive questions regarding your reproductive health. You are assured that only relevant details will be recorded in your history file, which will remain the property of the teaching centre. The file will be kept in a locked cabinet and only accessed by your teacher and such other persons as you authorise. You have the right to make an appointment to read your history file. If you wish to do so please discuss this with your teacher.
Such information as needs to be collected to meet the requirements of any government funding is de-identified.
All histories are kept for the statutory period of seven years after the file has ceased to be active after which time they are de-identified and destroyed.
